These are just a few of the most important foods, but you would do well to go further and improve your diet significantly. Read the list of foods to avoid for acid reflux (see above), and cut out all those foods immediately in order to improve your digestion. Diet for Acid Reflux Treatment: Good Eating Habits. Another thing to keep in mind is that you need to change the way you eat. If you eat large meals, you are filling your stomach and thus placing pressure on your lower esophageal sphincter. Stop eating such large portions, but eat fewer meals throughout the day. Large meals also stay in your stomach longer, thus increasing the amount of acid in your stomach. If you eat too close to your bed time, you will find that acid reflux will set in during the night. It is best to eat no less than 4 hours before you lay down for sleep, as that will ensure that your food is well digested. When eating out, make sure to avoid all the foods that are high in fat. You are probably going to have to eat out occasionally, so find a restaurant with foods that are healthy, natural, and low in fat in order to give you stomach a break. Home Remedies for Acid Reflux Treatment. There are some pretty awesome home remedies for acid reflex treatment, and these remedies will be great to try if you are feeling the pain from acid reflux, GERD, or heart burn: Aloe Vera – Aloe Vera is known for its calming and soothing effect, and it will help to soothe the pain in your esophagus and stomach. Papaya – As mentioned above, papaya can be effective in reducing the pain from acid reflux, but you may not have fresh papaya fruit if you don’t live in the tropics. The effects last for an hour or so before the acid starts building up. Barrett's esophagus. BARRETT'S ESOPHAGUS OVERVIEWThe esophagus is the tube that connects the mouth with the stomach (figure 1). Barrett's esophagus occurs when the normal cells that line the lower part of the esophagus (called squamous cells) are replaced by a different cell type (called intestinal cells). This process usually occurs as a result of repetitive damage to the inside of the esophagus caused by longstanding acid reflux disease, called gastroesophageal reflux disease (GERD). In people with GERD, the esophagus is repeatedly exposed to excessive amounts of stomach acid. Interestingly, the intestinal cells of Barrett's esophagus are more resistant to acid than squamous cells, suggesting that these cells may develop to protect the esophagus from acid exposure. The problem is that the intestinal cells have a risk of transforming into cancer cells. More detailed information about Barrett's esophagus is available separately. Children can develop Barrett's esophagus, but rarely before the age of five years. Gender — Men are more commonly diagnosed with Barrett's esophagus than women. Ethnic background — Barrett's esophagus is most common in white populations, less common in Hispanic populations, and uncommon in Asian and black populations. Lifestyle — Smokers are more commonly diagnosed with Barrett's esophagus than nonsmokers. BARRETT'S ESOPHAGUS SYMPTOMSBarrett's esophagus itself produces no symptoms. Instead, most people seek help because of symptoms of GERD, including heartburn, regurgitation of stomach contents, and, less commonly, difficulty swallowing. BARRETT'S ESOPHAGUS DIAGNOSISA healthcare provider may suspect Barrett's esophagus based upon a person's symptoms and the risk factors described above. An endoscopy is needed to confirm the abnormal esophageal lining. Upper endoscopy — Upper endoscopy is a test that allows your doctor to see the inside of the esophagus and stomach. Before the test, you are sedated to prevent discomfort. The doctor will insert a thin lighted tube into the esophagus. The tube has a camera, which allows the doctor to see the lining of the esophagus. Normally, the lining should appear pale and glossy; in a person with Barrett's esophagus, the lining appears pink or red and velvety. The doctor will remove a small sample of the lining (a biopsy) during the endoscopy so that it can be examined with a microscope for signs of Barrett's. Individual variations in the anatomy of the esophagus and the area where it meets the stomach can make the diagnosis of Barrett's esophagus difficult in some people. BARRETT'S ESOPHAGUS TREATMENTThe goal of treatment in patients with Barrett's esophagus is to control reflux symptoms. Aggressive reflux treatment may be more effective in preventing cancer than treating only when there are reflux symptoms. Most patients are advised to avoid certain foods and behaviors that increase the risk of reflux. Foods that can worsen reflux include. Carbonated beverages can be a problem for some people. Placing bricks or blocks under the head of the bed (to raise it by about six inches) help to keep acid in the stomach while sleeping. It is not helpful to use additional pillows under the head. Medications — A clinician may prescribe medications that reduce the amount of acid produced by the stomach. A class of medications called proton pump inhibitors is commonly recommended. Five different formulations (some of which are available as a generic) are currently available: omeprazole (Prilosec), esomeprazole (Nexium), lansoprazole (Prevacid), rabeprazole (Aciphex) and pantoprazole (Protonix); any of these is an acceptable option. Surgery — People who have severe reflux may benefit from surgical procedures to reduce reflux. Surgery is not the best treatment in all situations, so you should discuss this option with your doctor. More information about surgical treatments for reflux is available in a separate topic review. The early changes may progress to advanced precancerous changes, and finally to frank esophageal cancer. If undetected, this cancer can spread and invade surrounding tissues. However, progression to cancer is uncommon for any individual patient; studies that follow patients with Barrett's esophagus reveal that fewer than 0. Furthermore, patients with Barrett's esophagus appear to live approximately as long as people who are free of this condition. Patients often die of other causes before Barrett's esophagus progresses to cancer. BARRETT'S ESOPHAGUS MONITORINGMonitoring for precancerous changes is recommended for most patients with Barrett's esophagus. At this time, monitoring includes periodic endoscopy with tissue biopsy. Experts do not agree about the usefulness of monitoring. The benefits of monitoring depend upon each person's chance of developing esophageal cancer, which may be difficult to determine. Benefits — Reasons to perform endoscopic monitoring include. These changes may indicate that the person has an increased risk of cancer. Early detection may be especially important for younger patients. It is sometimes difficult to correctly identify precancerous changes, especially when there is inflammation (usually caused by the ongoing reflux of acid). Many clinicians increase the dose of acid- suppressing medications in this situation. The precancerous changes must then be graded as . The management of low- grade dysplasia is especially controversial. Some physicians recommend frequent endoscopic surveillance for patients with low- grade dysplasia, while others recommend destroying the abnormal tissue with radiofrequency ablation (see below). A person with high- grade dysplasia has more limited options. The management of this condition is controversial. The optimal treatment depends upon the person's age and health and the patient and physician's preference. The options include removal of the esophagus (esophagectomy) and removing (eg, endoscopic mucosal resection) or destroying (eg, radiofrequency ablation, photodynamic or other ablation therapies) the abnormal tissue using endoscopic techniques. Esophagectomy — In removing the esophagus, esophagectomy removes all of the precancerous tissue and some of the lymph nodes near the esophagus. However, this treatment has higher rates of procedure- related death and long- term complications than the endoscopic treatments for dysplasia. Esophagectomy is not necessary in most patients who have dysplasia in Barrett’s esophagus. In some patients, however, it may not be possible to destroy all of the abnormal tissue by endoscopic treatments, and esophagectomy may be recommended for those patients. Esophagectomy should be performed by an experienced physician in a hospital where the procedure is performed frequently. In one study of 3. Endoscopic mucosal resection — Endoscopic mucosal resection (EMR) involves the removal of a large but thin area of esophageal tissue through an endoscope. EMR provides large tissue specimens that can be examined by the pathologist to determine the character and extent of the abnormality and determine if an adequate amount of tissue was removed. Therefore, it can help to confirm the person's diagnosis and completely treat the abnormality (if the abnormal tissue is removed completely). However, this technique is generally performed only in specialized centers. Generally, EMR is performed if the endoscopist sees an area of nodularity in the Barrett’s esophagus. EMR is commonly followed by ablation of the remaining Barrett’s esophagus, usually with radiofrequency ablation (see below). Radiofrequency ablation — Radiofrequency ablation (RFA) is an endoscopic procedure that uses radiofrequency energy (microwaves) to destroy the Barrett’s cells. In short- term studies, RFA has been shown to prevent high- grade dysplasia from progressing to cancer and to prevent low- grade dysplasia from developing more advanced features. However, there is limited information on the long- term outcome of this approach. In up to 5 percent of patients, the procedure causes a complication, such as narrowing of the esophagus, which may require repeated treatments to open the esophagus. Another concern with RFA is that, in a small minority of patients with high- grade dysplasia (less than 2 percent), there may be cancer in the lymph nodes adjacent to the esophagus. RFA cannot cure cancer in the lymph nodes. In all cases, the patient and family should discuss the risks and benefits of possible treatments with a healthcare provider. Photodynamic therapy — Photodynamic therapy is a treatment that uses chemical agents, known as photosensitizers, to kill certain types of cells (such as Barrett's cells) when the cells are exposed to laser light. Patients are given the photosensitizer medication into a vein and then undergo endoscopy. During the endoscopy, a laser light is used to activate the photosensitizer and destroy the Barrett's tissue. However, there is limited information on the long- term outcome of this approach. Furthermore, photodynamic therapy is expensive and available in only a small number of academic medical centers. In up to 4. 0 percent of patients, the procedure causes a complication, such as narrowing of the esophagus, which may require repeated treatments to open the esophagus. Another concern with photodynamic therapy is that patients with high- grade dysplasia may have areas of invasive cancer that are not treated adequately. Photodynamic therapy has largely been replaced by RFA, which appears to be safer and at least as effective. In all cases, the patient and family should discuss the risks and benefits of possible treatments with a healthcare provider. SUMMARYDespite the uncertainties surrounding the monitoring and treatment of Barrett's esophagus, there is consensus on one matter: The available options should be tailored to the individual patient. The following are general guidelines. By incorporating intense periods of work with short recovery segments, interval training helps you give maximum intensity while still maintaining your exercise form. The magic of high intensity interval training (or HIIT, for short) is that it keeps your body burning fat even after you leave the gym. During a HIIT workout, your body can't shuttle enough oxygen to your muscles during periods of hard work. Therefore, your muscles accumulate a “debt” of oxygen that must be repaid post- workout in order to get back to normal. The result: Your metabolism is revved for hours after you leave the gym. Trainers refer to this phenomena as excess post- exercise oxygen consumption, or EPOC. The biggest way to use it to your advantage is to regularly work short, intense bouts of exercise into your workout regimen. Intense circuits also stimulate muscle- building hormones like growth hormone and IGF- 1. This puts your body in a perfect state to build lean mass. And in addition to the hormone response, interval training also develops the cardiovascular system. By elevating your heart rate during periods of hard work, you’ll increase your cardio ability and strengthen your heart. During the short rest intervals, you also increase your recovery capabilities—meaning you’ll be able to recover faster in future workout sessions. When creating interval workouts, focus on including difficult movements that challenge your entire body in a single exercise. Aim to include at least two exercises back to back before allowing your body to recover. Catch your breath in between rounds, but don’t allow yourself to recover fully before attacking the next set. It was going through this process that sparked my renewed interest in the health and fitness community and snow balled into me being much more passionate about all this. If you’re looking to lose weight quickly, there are three things you need to optimize or focus on: Your diet – what you eat and drink. Your exercise – timing and duration of specific exercises. Supplementation – speeding up the process with the help of safe drugs. I really think that the best way to lose weight is by optimizing your diet. More specific it’s to manage your blood sugar levels and eat for your specific metabolism. I lost my weight by only optimizing my diet. I changed what I ate dramatically and literally melted the weight off. I remember waking up on the fourth day after starting this in complete amazement by the fact that my fingers felt skinny. I’m not sure if it was the amount of salt I ate or what, but my fingers and hands have felt fat for the last year – this alone was enough to keep me going. For the sake of this overview, I will not go into any detail on supplements (as I have yet to learn much about them), and will only briefly touch on exercising. Instead, I’ll focus on optimizing diet for rapid weight loss. So how did I lose this weight so quickly? I kept my blood sugar from spiking by following a fat loss program built for me. It was quite similar to the slow carb diet. The methods I used were from the advice I found from a variety of locations, surrounding foods and how diet alone can make for some amazing weight loss. These were the key changes I made that I know helped with the rapid weight loss. I ate a big, high- protein breakfast. I love my breakfasts. It’s crazy to think that I could lose 1. It’s great though because in the evening when I sometimes get cravings, I will just remind myself how awesome breakfast is going to be. I included a picture to the right of a typical breakfast that I will eat. It’s three fried eggs, 4- 6 pieces of bacon, and fried mushrooms, onions, and beans. Delicious. Lets be honest, there were mornings that I just didn’t have it in me to make this. For those mornings I would fall back to just 2- 3 hardboiled eggs, or a single hardboiled egg and a protein shake made with water + whey protein isolate. Since I work from home, I can typically take the time to make a big breakfast, but probably 1/6 days I would have to fall back to the quick breakfast. Remove most dairy products. This wasn’t a huge issue for me. I haven’t drank much milk since junior high, and although I do enjoy cheese on almost everything it was easy enough to give up. Luckily, I could still use butter to cook with, and if you are like the many people that love cream in their coffee – this is okay as well as long as it’s heavy cream and not milk. Cheese is a big part of my cheat days as I explain below. Water, Coffee, or Tea. This was one of the biggest challenges for me. That said it is crazy how fast you can lose weight quickly if you simply cut out drinking unnecessary calories. Admittedly, I’ve historically enjoyed a beer or two over dinner at home – and even more than one or two if out with some friends. Although I don’t think there’s anything wrong with a beer a night, or a glass of wine a night, I know the former is not great for losing weight. If all you have is two weeks to lose weight, you’re going to have to be very strict with this rule. Luckily you can still drink any water- based drink such as coffee and tea, so it’s not all bad. I usually have a huge glass of ice cold water plus one or two large cups of coffee in the morning to kick start my day. I often use cinnamon to flavour my coffee – it’s a great tip for those of you that really enjoyed adding milk. No white carbohydrates. In fact, anything that is white or can be white. So no pastas, no rice, no breads, no oatmeal, no potatoes, etc. However, I still ate a ton of carbohydrates. I just ate better high- fiber carbs such as black beans, kidney beans, and lentils. The latter I didn’t really enjoy, so most of my meals were made up of beans. The picture to the left is an example of what I would eat for supper. This night I had a spread of black beans, cooked frozen veggies, and sliced ham. I basically substituted the ham for any other meat – chicken, beef, or pork mostly – and alternated between black and kidney beans. Note – if you find your food getting bland, try using mustard and pepper. For lunch, I’ll often take 1/2- 1 can of tuna, 1- 2 cups of frozen veggies, and a big scoop of bean, cook in the microwave, add a couple squeezes of mustard and pepper and shake. Moderate 2. 0 minute weight resistance twice per week. When I was reading the Four Hour Body, it really got me interested in how the body works and how it deals with exercise, food, etc. This is one thing that I think really helped me – especially during my cheat days. I will do a light to moderate 2. Bicep curl x 1. 5Air squats x 5. Military shoulder press x 1. Wide arm pushups x 2. Against- the- wall tricep extensions. As many close- grip pull ups as possible. Calf raises x 3. 0The goal of the workout was to promote food storage to muscle instead of fat – this is especially important during your cheat days. During my cheat day I will look to do the air squats + tricep extensions even if I’m eating out. I didn’t go to the gym at all for this, instead I just took a couple old 2. If you don’t have a pull up bar don’t buy one, simply exchange the exercise for a back row or seated back fly exercise. I also don’t own a weight bench so no need to spend your money on that. What I’ve learned since losing even more weight is that these workouts are pretty important if you want to lose weight fast. They aren’t necessary but they certainly do help shed some extra pounds by keeping the fat off, especially on your cheat day. I gave myself one cheat day. I say one cheat day, because I wanted to illustrate the fact that this diet can be used for people wanting to know how to lose weight in 2 weeks. If you are only using this as a fast way to lose weight, then you’ll only want to indulge in one cheat day during your two- week period. However, I now give myself every Saturday as a cheat day. I believe that this cheat day is important for a number of reasons. Firstly, the science behind it suggests that if you go for too long on a limited- calorie diet (which this isn’t, however I noticed that it took me much, much less food to fill me up when I ate well, so you tend to naturally eat less on this diet) that your metabolism will shift to a lower gear in order to compromise for the lower intake of calories. By cheating one day and spiking the number of calories you consume, it will keep your metabolism from doing this – and allow you to maintain your high metabolic rate throughout the week. Personally, I find the cheat day important because it gives you something to look forward to. This diet is a huge change from most Westerners’ lifestyles. With the diet comes a lot of cravings, especially in the beginning. I would make a little note of anything that I was craving and made sure I went out and bought it Saturday morning. For example, this is what my first cheat day looked like: Breakfast – bacon and eggs with hash- browns, toast, a ton of ketchup, and a fruit smoothie. Snack – Full bag of All Dressed potato chips and a large bag of sugar candy (Cherry Blasters)Lunch – Entire box of extra cheesy Kraft Dinner. Snack – 2 large Caramilk chocolate bars. Supper (and evening snack) – 1/2 large poutine, a few garlic fingers and 7 slices of a 1. |
